As a Band 6 Diabetes Specialist Nurse, you will: Take ownership of your caseload
as an autonomous and accountable practitioner, proactively monitoring and anticipating the changing needs of people with diabetes. Deliver expert care
by performing high-level assessments, agreeing on personalised treatment plans, and initiating injectable therapies. Empower patients to thrive , supporting them to achieve independence or reach their maximum potential through education, shared decision-making, and seamless collaboration across care teams. Work independently, with support from senior diabetes specialists , to achieve exceptional quality and performance standards. Collaborate with stakeholders across the system , maintaining high standards of care while delivering impactful education and training to both colleagues and service users.
Main duties of the job
As a key member of our team, you'll deliver advanced clinical management, utilising specialist skills and knowledge to transform patient outcomes.
In this role, you will: Deliver nurse led clinics to people living with diabetes, providing expert diabetes care and education , equipping patients with the tools and confidence to take control of their condition and embrace self-management. Develop personalised treatment plans
in partnership with the person with diabetes that not only address immediate needs but also proactively prevent chronic complications, promoting long-term wellness. Be at the forefront of innovative care , reducing unnecessary hospital admissions through advanced management of acute exacerbations and providing early supported discharge for patients transitioning from secondary care.

Job responsibilities To work as an accountable and autonomous practitioner working to pro-actively monitor and anticipate changing needs of patients with diabetes. To provide advanced clinical care using high level assessment to make a diagnosis, initiate treatments, provide education and refer appropriately as needed. Improve clinical outcomes for patients by enabling them to function independently by increasing their choices, providing expertise to enable informed decision making. To work effectively as an independent practitioner supported by the Senior Clinical Nurse Specialist in diabetes to meet agreed quality and performance targets. To work closely with both primary and secondary care to ensure a safe and effective patient pathway is in place for all diabetes patients under the care of the community diabetes team.

Essential First level nurse qualification NMC Diploma in diabetes care or equivalent qualification or willing to work towards Teaching qualification eg FLIPs / ENB 998 Experience working with patients with diabetes
