Are you an experienced registered nurse with a passion for diabetes? We have an exciting opportunity to work for the Diabetes Intermediate Care Service as a band 6 registered nurse. There will be training provided for the newly appointed candidate to help develop their role to enable them to work autonomously as a specialist diabetes nurse. A full driving licence and the use of a vehicle is essential for this countywide service. We offer a generous mileage allowance to cover the use of your car for NHS business, which includes both mileage and depreciation. You will require excellent communication skills, a teamwork approach, and be able to provide a high standard of nursing care with the support of your team. Main duties of the job

As part of the Diabetes Intermediate Care Service, you will be responsible for the delivery of safe and effective care for your own caseload of patients in Somerset. Communicate with patients, colleagues, internal and external contacts in a courteous, professional and timely manner at all times by various methods, i.e. telephone, written, face to face or virtual consultation. Participate in team and Trust meetings as required. Assist with mentorship, training and induction of newly appointed staff. Report any incidents by way of the Trust Incident Reporting system. To provide and receive complex, sensitive and possibly contentious information where highly developed interpersonal and communication skills may be needed. To contribute to projects in relation to the development of the Trusts Corporate Service Plan and organisational objectives as required. Provide group education to people newly diagnosed with type 2 diabetes such as DESMOND.

Job description

As a Diabetes Specialist Nurse within the Intermediate care service the post holder will be responsible for delivery of safe and effective care and support of patients living with Diabetes. Educational provision in the care of diabetes, both internally and externally to the Trust is equally an essential element. There is an expectation to support the County wide model of care whilst holding a case load within a specific locality linking with community neighbourhoods. We are currently embarking on an exciting journey to integrate patient pathways with our acute colleagues and the post will require a willingness to proactively accept and contribute to change.

Person Specification

Qualifications

Essential Good standard of general education Must be able to communicate in English Language, both written and verbally appropriate to the post Registered Nurse. Evidence of Teaching qualification for example ENB998 Working towards a Degree or equivalent knowledge

Experience

Essential Previous NHS experience (minimum 3 years) Evidence of supervisory and mentoring skills Significant experience of working with people with diabetes. Experience in working as autonomous practitioner
