An exciting opportunity has arisen for an enthusiastic and highly motivated nurse to join the Wirral Community Diabetes Service. This Community Diabetes Specialist Nurse will provide highly specialist Diabetes care to patients who have Diabetes Mellitus using advanced clinical knowledge. The successful candidate will work collaboratively with other health care professionals within the Community Diabetes Service to provide excellent diabetes care and meet the needs of this patient group. This 8-hour post is based within the community, but the post holder is fully expected to work closely with other members of the Wirral University Teaching Hospital (WUTH) Diabetes MDT. Main duties of the job
To request and interpret diagnostic investigations. Make clinical judgements, adjusting treatment plans according to clinical need. Acts independently as a registered Nurse Prescriber (V300) for patients with Diabetes mellitus. Prescribes medication within an agreed clinical management plan with the patient's agreement, making highly complex decisions to improve patient outcome. To develop, produce and implement diabetes mellitus protocols which impact across primary and secondary care. Provides highly specialist advice including knowledge of diabetes structured education and sensor technology. Provide one to one counselling to patients on life changing treatments, ensuring patients understand complex drug therapies and chronic disease management. Provide a holistic, highly autonomous specialist nursing service to patients with Diabetes Mellitus assessing physical, psychological and social needs and implementing an agreed clinical management plan with the patient. Registered General Nurse Significant relevant experience in Diabetes specialty and experience of Diabetes Structured Education and Insulin Pump and Sensor technology Independent prescribing course (V300) Insulin Pump Course (or willingness to undertake) Master's level (or working towards) or equivalent experience Essential Teaching and assessing in Clinical Practice (ENB 998) Experience of working with young people and pregnant ladies with diabetes Knowledge and Skills
