Main Duties of the Job
Provide leadership in the field of diabetes and draw on advanced knowledge and exercise professional skills to develop and deliver high quality service to people with diabetes.
Act as an expert practitioner, overseeing, delegating and supporting care carried out by other members of the diabetes team where required.
Work closely with other departmental staff and professionals to facilitate a multidisciplinary framework for the effective delivery of diabetes care.
Work collaboratively and co-operatively with clinical colleagues to develop an innovative high quality service, which enables individualised care to be facilitated and delivered to people with diabetes and their carers/families.
Responsible for delivering an equitable nurse-led service for people with diabetes in Warrington and Halton.
Be responsible for a defined clinical caseload of people living with Type 1 and Type 2 diabetes.
Make an assessment on complex facts to formulate a care plan, implement and continually evaluate care.
Review patient’s medication regime ensuring that patients receive appropriate pharmacotherapy in effective doses. Prescribe changes or initiate medication in accordance with non-medical prescribing.
Refer people to relevant practitioners or agencies when this is necessary providing accurate and necessary information consistent with maintaining confidentiality.
