We are looking for an individual that would like to work as band 3 Discharge Co-ordinator, to work within Coastal Lodge Inpatient Rehab Team. You will work with the inpatients, their next of kin and our MDT teams to support and facilitate early discharge planning to avoid any delay.

To build a deeper understanding of patient need, through conversation as soon after admission as possible, regarding patients home environment and family circumstances, bringing in the right expertise at the right time and seeking innovative solutions.

To clearly set expectations around the discharge journey from hospital to patients and their relatives.

This is a 30 hour contract which we are happy to look at flexibly over the week but individuals will need to be visible and able to attend morning handover, MDT meetings or meet with next of kin and families at mutually agreeable times within Coastal Lodge.

Applications from candidates that require current Skilled worker sponsorship to work in the UK are unfortunately ineligible to apply for this position.

You will be expected to work in the Coastal Lodge environment as a Discharge Co-Ordinator alongside the MDT to ensure that effective admission and discharge processes are undertaken.

Ensure effective communication with the MDT, social services, cluster leads, SPA and voluntary agencies regarding simple and complex needs identified during the admission process to provide effective discharge planning to patients and their representatives that facilitates timely discharge.

To ensure all appropriate literature is disseminated and understood by patients and their next-of-kin's. This will assist in setting their expectations and highlight any concerns early in their admission.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
