We are looking for a dynamic nurse/physio/OT to work in the Discharge management team working across all CDDFT hospital sites.

Main duties of the job Proactively advise on the planning and coordination of complex discharges within the Trust. Carry out trusted assessments within ward areas, completing appropriate documentation. Case load holder for patients requiring extra care. To review the medically optimised list and work with staff to keep this updated with correct information. Support joint assessment, joint documentation and best practice to support proactive discharge planning across the Trust through education and training for all ward staff. To support ward discharge facilitators to enable safe timely discharge and minimise the risk of unnecessary re-admission to hospital. Provide support, education and specialist knowledge on all aspects of discharge planning to staff working within the Trust. Liaise with ward staff and discharge facilitators to identify and problem solve issues for patient discharge. Escalate delays to senior nurse where delays are impacting upon the discharge process. To establish effective relationships with multi-disciplinary ward teams, Continuing Health Care, Social Services, community teams, Care Home selection Team, Primary Care colleagues, Housing and other relevant departments.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Job description Job responsibilities

To work as a member of the discharge Management team, promoting safe and effective discharge and ensuring consistency of discharge management process across the Trust. Key to this is the identification of issues relating to complex discharge, expert advice and supporting ward discharge facilitators to enable safe timely discharge and minimising the risk of unnecessary re-admission to hospital. The Discharge Management practitioner will be responsible for providing education and training to ward staff for the discharge management process, pathways and policies.

This post aims to improve the quality of discharge from hospital through close multi-disciplinary and multi-agency working relationships. The discharge practitioner will work within the discharge management team to adopt a whole systems approach to discharge management and acts as a catalyst for improved working at the primary, secondary and community interface.

Person Specification Qualifications Essential BSc/MSc in Nursing/Physiotherapy/Occupational Therapy Evidence of CPD, including attendance at recent post-graduate courses relevant to the clinical fields in the specialism

Desirable Management development course Knowledge of budgetary control

Experience Essential Experience of change management Community/hospital experience Experience of clinical supervision

Desirable Experience in staff appraisals Experience in working in a variety of settings community and Acute

Statutory Registration Essential 1st Level Registration NMC/HCPC

Desirable Member of NMC/CSP/RCOT

Special Skills & Knowledge Essential In depth and up to date knowledge and skills in the assessment and treatment of a wide range of physical disabilities/Health conditions. Remain updated with professional practice and new research. Understand clinical governance and its implication for services, including experiences of quality and audit. Ability to comprehend and work within the Trust's policies of data protections, equal opportunities and health and safety. To meet the differing needs of patients. Ability to carry out moderate to intense physical effort throughout the working day and carry out concurrent activities. Ability to cope with working in stressful environment and deal with emotional or aggressive patients and carers. Ability to work independently and organise, prioritise and delegate workload to meet service demands. Understand the legal responsibilities of the profession. Evidence of self-directed learning and critical appraisal skills.

Special Requirements Essential Ability to travel and carry small items of equipment on a daily basis to fulfil the requirements of the post.
