To work as a member of the discharge Management team, promoting safe and effective discharge and ensuring consistency of discharge management process across the Trust. Key to this is the identification of issues relating to complex discharge, expert advice, and supporting ward discharge facilitators to enable safe, timely discharge while minimizing the risk of unnecessary re-admission to hospital. The Discharge Management Practitioner will be responsible for providing education and training to ward staff regarding the discharge management process, pathways, and policies. This post aims to improve the quality of discharge from hospital through close multi-disciplinary and multi-agency working relationships. The discharge practitioner will work within the discharge management team to adopt a whole systems approach to discharge management and act as a catalyst for improved working at the primary, secondary, and community interface.
