An exciting opportunity has arisen within the Discharge Team. You will work as part of a team coordinating referrals received from our wards and sending them to the appropriate service following a validation process. You will help improve patient flow, primarily working at Poole Hospital. The role will incorporate cross-site working and cover with Bournemouth Hospital. You will be working closely with our partners and building good working relationships. The role is full-time and covers a 7-day working pattern.
