We are a well-led, high-performing, and innovative organisation determined to provide the best health care services for our local population and working in the best interests of the people we serve. Job overview
An exciting opportunity to work within a developing and expanding team has arisen. The Discharge to Assess (D2A) team is a team of Therapists who work with people within their homes on discharge from Hospital. This is to ensure patients' needs are being met at home and provide an opportunity for more accurate assessment in their own homes, compared to the unfamiliar setting of an acute hospital. The Therapist (Occupational Therapist or Physiotherapist) will need to work holistically within the MDT and utilise their profession-specific skill-set to deliver high-quality care and support the patient in their own home. Experience of working in the acute or a community setting will be helpful. The role will involve working with multiple agencies, inside and outside of the hospital, to provide the best service for the people in our care. MSE has a focus on developing the clinical and non-clinical skills of its staff both personally and professionally. Main duties of the job
