There have been many changes in how patients receive their health care, and this is ever increasingly provided in the community setting. It is definitely the place to gain skills and development in Case Management, Physical Assessment, Prescribing, managing Long Term Conditions, and many Clinical Skills.
Development of your leadership skills is essential, and we will provide the necessary training and support to achieve this.
Supporting Junior and new staff is very much part of the role, and you need to have Practice Assessor/Supervisor experience to support the different levels of students.
The role includes using your clinical skills for assessment, diagnosis, treatment, and/or appropriate referral in collaboration with other agencies to ensure care coordination, and includes being named as Key Link for individual practice(s). This also includes attending GSF, MDTs, Individual Patient Reviews, Hospital Discharge Planning, and CHC Assessment and Reviews.
As well as the GPs and Social Services, you will also be expected to develop and maintain relationships with Nurse Specialists within SCFT, our partners in Palliative Care, Sussex Partnership NHS Foundation Trust, and the voluntary sector in our community.
