We have an exciting opportunity to recruit to the post of 'Enhanced Clinical Practitioner' to work in the community looking after our frail and elderly patients. The role is a full-time permanent position working 37.5 hours per week. Must have completed both level 7 modules of APACS/CDM and Non-Medical Prescribing qualification and completion of E-portfolio to be uplifted from a trainee band 6 to a qualified band 7. Or – able to evidence all aspects of person spec if already in same role. Main duties of the job

To provide expert enhanced clinical management for frail patients and those with multiple long-term conditions, at risk of deteriorating health. To conduct enhanced clinical assessments, make diagnoses, initiate appropriate treatments, and coordinate care with partner agencies. Investigate and diagnose unwell individuals in their homes and care homes. Reduce unplanned admissions when appropriate. Manage long-term conditions, including frailty. Provide high-quality care to patients and families. Manage complex medical conditions. Carry out acute one-off visits when required (within own skill set). Handle challenging behaviour and work with existing resources. Support vulnerable clients and families. Develop clinical skills. Working for our organisation

Education and Qualifications

RGN or HCPC accredited degree that can complete the additional post registration training to V300 Independent non-medical prescribers. Registered with NMC (Nursing and Midwifery Council). Evidence of post registration clinical professional development. Enhanced clinical decision-making and patient examination skills. Personal Attributes and qualities

Able to recognise own limitations, identify learning needs, and act on them appropriately. Ability to work autonomously and use own initiative. Ability to assess and manage risk when working with individuals. Experience of working with patients in their own homes or a care setting. Flexible approach to the evolving nature of the role. Ability to work collaboratively in a team. Drive to provide excellent integrated patient care. Ability to work under pressure to meet deadlines. Adaptable approach to meet service needs. Experience

High level of understanding regarding long-term conditions agenda and wider NHS agenda. Experience working autonomously in an advanced nursing role. Community experience. Previous experience of Leadership/Management. Experience in a primary care setting. Experience with SystmOne. Experience or training in personalised care and support planning. Skills and Knowledge

Excellent communication skills, both written and verbal. Management of risk and health and safety, and safeguarding adults and children procedures. High-level clinical skills relevant to long-term conditions and chronic disease management. Advanced clinical practice skills. Management of patients with complex needs. Accountability of own role and other roles in a primary care setting. Knowledge of practical application of clinical governance. Willingness to work flexible hours when required to meet work demands. Access to own transport. Ability to travel across the locality on a regular basis. Diagnostic and clinical assessment skills. Teaching and mentorship in a clinical setting.
