Job overview

We have a fantastic opportunity for an Enhanced Clinical Practitioner (ECP) to work in the Swadlincote Acute Home visiting service. The Acute Home Visiting service works collaboratively with the local GP and Integrated Teams in Swadlincote, providing support to patients in their own homes and in the Oakland bedded unit which supports patients discharged from hospital. Our team is happy, skilled, and provides an excellent working environment with opportunities to learn and develop in this role. You will improve clinical outcomes for patients by enabling them to function independently, increasing their choice to remain in their own home/community, and reducing the need for, or preventing inappropriate hospital admission. We aim to recruit clinicians who are passionate about supporting and empowering patients and their family/carers to manage their complex health needs. We strive to support patients in accessing help when needed and optimising health outcomes. Education/Qualifications

RGN or HCPC accredited degree Evidence of post registration clinical professional development V300 Independent Non-Medical Prescriber Clinical Decision Making course at Level 7 or equivalent Teaching qualification Experience and Knowledge

Knowledge of practical application of clinical governance High level of understanding regarding long term conditions agenda and wider NHS agenda Management of risk and health and safety, plus safeguarding adults and children procedures and ability to implement Previous community experience Skills

High level clinical skills relevant to long term conditions and the management of chronic disease management Excellent communication skills, written and verbal Diagnostic and clinical assessment skills Experience of using IT patient records in Primary Care Other

The post holder will need to have appropriate transport to be able to visit patients in their own home throughout shifts, carrying appropriate equipment.
