We have a fantastic opportunity for an Enhanced Clinical Practitioner (ECP) to work in the Swadlincote Acute Home Visiting service.

The Acute Home Visiting service works collaboratively with the local GP and Integrated Teams in Swadlincote, providing a varied source of support. Our patient group consists of patients in their own homes and in the Oakland bedded unit, which supports patients discharged from hospital.

Our team is happy, skilled, and provides an excellent working environment with opportunities to learn and develop in this role.

You will improve clinical outcomes for patients by enabling them to function independently, increasing their choice to remain in their own home/community, and reducing the need for, or preventing inappropriate hospital admissions.

We aim to recruit clinicians who are passionate about supporting and empowering patients and their family/carers to manage their complex health needs.

We strive to support patients in accessing help when needed and optimizing health outcomes.

We want to support patients to remain in their own homes as appropriate, preventing unnecessary admissions to hospital, and to support patients to return home as needed.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes, and increasingly, in people’s own homes and via virtual consultations.
