Greater Middlesbrough Primary Care Network (PCN)

The successful candidate will be part of a dynamic and forward-thinking PCN led Multi-Disciplinary Team (MDT). Clinical leadership will be provided by the PCN Clinical Directors and Clinical Leads.
The successful candidate will be based in a local cluster of General Practices as part of the PCN.
We are looking for individuals who will lead clinical care home rounds & coordinate the work of healthcare professionals and non-clinical staff in the care of patients registered at GP practices within the wider PCN population. The role includes responsibility to work with the PCN Operational Manager and the practices to support and ensure compliance with the reporting requirements of the PCN Directed Enhanced Services (DES).

Main duties of the job The role will involve working across organisational & professional boundaries to provide clinical care and leadership and co-ordinate the treatment, care, management, and empowerment of care home residents.
The individual will take part in the weekly care home rounds to include:
Lead weekly ward rounds in our aligned Care Homes
Weekly MDT's with our Learning Disability EHiCH colleagues in our LD Care Homes
Review medication
The Enhanced Practice Nurse will work closely with the Care Coordinators, GPs, and practice teams to manage a caseload of patients, acting as a central point of contact to ensure appropriate support is made available to them and their carers; supporting them to understand and manage their condition and ensuring their changing needs are addressed.

Greater Middlesbrough PCN consists of multidisciplinary teams that currently include Clinical Pharmacists, Care Coordinators, Mental Health Practitioners, First Contact Physio, and Social Prescribing Link Workers.
We are an active network and closely work with our local voluntary organisations and other partners to develop new models of care to benefit our patients.
The role will provide additional flexibility to the PCN by adding to the growing expansion of the primary care workforce.

Person Specification Experience
Experience of collaboratively working at a senior level with all health, social care and voluntary sector providers, primary and secondary care professionals, care homes, palliative care services, patients, carers, and commissioners
Experience of supervising and appraising staff
Experience of working collaboratively with other organisations
Experience of clinical audit, evaluation research and implementation of evidence-based practice
Registered Nurse with qualifications relevant to enhanced practice
Experience in delivering enhanced clinical care as part of a MDT within the PCN
Knowledge and Skills
Knowledge of the workings of general practice
Awareness of the NHS Modernisation Agenda
Willingness to undertake training at a higher level if necessary
Willingness to participate in clinical supervision and performance review
Excellent interpersonal and communication skills
Ability to work effectively in a multidisciplinary team as a key player
Effective problem-solving and decision-making skills
Information technology skills including Word, Excel and electronic patient record systems
Personal
Commitment to ongoing personal and professional development
Ability to be self-directed and self-motivated
Ability to work alongside, lead and motivate the team
Ability to work effectively with other members of the team and other partners and agencies
Contractual Requirements
A full UK driving licence and vehicle for business use is required for this post; however, reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010
