The MSK physiotherapy service has worked closely with County Durham CCG to commission an integrated MSK service for County Durham patients (CD IMS). The CD IMS pathway provides a single point of access to the full spectrum of MSK services from routine physiotherapy to specialist physiotherapy and orthopaedics. Main duties of the job

All patients referred will be triaged by an ESP, who will direct patients to the right clinician the first time. This may include physiotherapy, ESP opinion for diagnostics, onward secondary care referral, or AHP referral. As an ESP, you are responsible for delivering a leading standard of musculoskeletal physiotherapy assessment, triage, and care to a varied selection of NHS patients. You will work as a Frontline ESP in various locations, as part of a wider integrated community physiotherapy service. You will be required to perform frontline patient screening with direct referrals from GPs, care navigation, and self-referrals; using expert assessment and diagnosis to determine which clinical pathway will be most suitable for the patient. You will also be required to provide expert clinical assessment, diagnosis, and management of your caseload. From initial assessment, you will be responsible and accountable for devising, reviewing, and completing appropriate treatment management programs to the patient population, in order to gain successful clinical outcomes, in line with your regulatory body's standards of professional practice. You must be able to demonstrate excellent assessment and clinical reasoning skills that this post will require, within a variety of musculoskeletal, orthopedic, rheumatology, and general health conditions. You should possess highly developed differential diagnostic skills to ensure patients are managed safely, efficiently, and effectively. Good communication skills with a variety of people, conditions, and within demanding situations will be an asset. Clear evidence of team working and the ability to develop effective and efficient services is essential. Ability to direct the care pathway for your patients autonomously, including assessment and diagnostics, provision of diagnosis, appropriate self-care advice, or onward referral as indicated. Familiarity with the IRMER, when and which diagnostics are suitable, and confidence in explaining their use and implications to patients is required. You should demonstrate commitment to the role with a strong desire to continually improve as a musculoskeletal physiotherapist, developing a culture that improves quality by initiating, promoting, and monitoring systems which govern and improve quality. You will be joining a friendly team with other physiotherapists who can offer a varied caseload with patients from primary and secondary care. Develop clinical reasoning skills with our triage team for both online self-referral and single point of access patients. Opportunities to assist with student education, from lecturing to supervising students on clinical placements are available. A structured and advanced in-service training program will be provided, along with regular formal/informal supervision to monitor and help you achieve your goals and aspirations within Physiotherapy. HCPC Member of the CSP (Chartered Society of Physiotherapy) Qualifications

BSc Physiotherapy / diploma Recognised relevant academic post qualification in relation to MSK conditions to masters level Continuing Professional Development Diary including evidence of reflective practice, training, and clinical experience Injection therapy competent Significant experience at a senior level in MSK physiotherapy An understanding of County Durham and Darlington NHS Foundation Trust, its culture, strategic direction, achievements, and NHS standing Special Skills & Knowledge

Up to date knowledge of professional practice Evidence of comprehensive and up to date range of highly specialised clinical skills An understanding of legal responsibilities within the profession Able to present information, written and orally in a clear and logical manner Ability to organise and prioritise and work to 18 RTT. Evidence of critical appraisal in research and evidence-based practice Ability to maintain high levels of concentration and analysis of presenting symptoms from patients Excellent communication skills both written and verbal Excellent presentation skills Innovative approach to working life Able to use own initiative appropriately to enhance service delivery Evidence of strategic planning for MSK services Special Requirements

Able to travel across Trust sites to meet the requirements of the post Experience

Comprehensive experience including core competencies within musculoskeletal, neurology pain management Evidence of being able to lead and improve clinical practice Experience of Cognitive Behavioural Therapy and Bio-psycho-social model of care Disclosure and Barring Service Check
