This is a new and exciting role within CHCP, to work in a rehabilitation unit setting providing high quality advice to patients identified for rehabilitation.
As an exercise practitioner, you will work flexibly across the bedded units within the CHCP portfolio providing expertise in exercise prescription, integrating exercise prescription and progression in patients' rehabilitation programme whilst taking into consideration patients' comorbidities. You will consider the functionality of movement and meaningfulness of activities to patients' holistic rehabilitation journey.
Main duties of the job
You will be supported by well-established MDT teams who provide rehabilitation within intermediate care, this includes Occupational therapists, Physiotherapists, and associate practitioners primarily.
You will receive clinical supervision from senior clinicians within the service.

An understanding of the needs of socially and culturally excluded groups of patients Excellent written and verbal communication skills Able to speak in a clear and articulate manner, dealing with people in a courteous way Able to clearly communicate sensitive and emotive issues to patients that may receive bad news Hold and complete good quality, error-free clinical documentation and records in a timely manner Good time management, planning, and organizational skills Ability to review exercise data and information to ensure effectiveness of individual risk stratification programme Able to work independently using own initiative and as part of a multidisciplinary team Able to assist with supervising students on placement within the team Experience of using Microsoft Office Tools (to include - Word, Excel, PowerPoint, Outlook, Intranet, and Internet)
