Job Brief

To contribute effectively and professionally to the work of the '0 -25 Together' Service by providing appropriate responses to the children, young people, young adults and their families in need of support and taking appropriate safeguarding action as and when required. More broadly to support the delivery of the '0-25 Together' vision for a specialist service for disabled children, young people, young adults and their families with a focus on supporting people to reach their full potential, enabling their independence and ensuring that they can take part in their local community. The service will support young people and young adults until they are settled in adult life.

Main Areas of Responsibility

Responding to Need

Provides professional response in situations where children, young people and adults need support. Using skills to inform trusted assessments, decision making and interventions.

Trusted Assessments: Undertakes proportionate assessment of vulnerable children, young people and adults in accordance with current Children or Adult Social Care legislation.

Completes care and support planning with children, young people and adults and their families to ensure the outcomes identified at assessment are met through the provision of support, equipment, adaptations.

Undertakes Mental Capacity assessments and Best interest decisions where appropriate.

Actively engages with, assists and provides advice to carers, conducts carer's assessments to enable them to sustain their caring role.

Direct work with children/ young people who are going through the safeguarding or court process.

Ensuring effective safeguarding and risk management

Identifies and takes appropriate safeguarding action (as determined by legislation and current HCC policy).

Supports lead officers in safeguarding process.

Effectively assesses and manages risk.

Supports safeguarding investigations under the direction of a manager, social worker, occupational therapist or nurse.

Partnership Working

To develop effective professional networks with partner agencies eg schools, colleges, youth connexions, health, housing and leisure to best support the child, young person, young adult to reach their full potential, enabling their independence and ensuring that they can take part in their local community.

Develop, nurture and sustain links with settings in your locality area.

Promote independence & autonomy in preparation for adulthood

Provides support where a person is constrained by social or family circumstances and provides support to achieve a reasonable degree of independence and autonomy.

Uses a person-centred approach, developing creative and personalised solutions to assist children and young adults to manage their lives independently.

Promotes independence and community wellbeing, choice and control within a personalisation framework utilising universal services and flexibility via the use of self-directed support.

Demonstrate HCC Values and Behaviours and Professional Standards

Carries out duties in a timely and responsive manner, in line with HCC's Values and Behaviours framework.

Maximises efficiencies in care planning through consideration of charging systems and other funding streams.

Keeps and maintains accurate, up to date service user records, in line with professional requirements and departmental recording methods.

Qualifications and Knowledge:

Essential Criteria

Clear knowledge and understanding of social and / or health care within the community.

Desirable Criteria

A qualification in care work or relevant other qualification.

Human rights legislation and how they protect the rights of adults.

The integration agenda - Working as part of a multi-disciplinary /multiagency team.

The personalisation agenda and applying creative problem solving to maximise independence.

Relevant Demonstrable Experience:

Essential Criteria

Experience of working within a paid or voluntary care setting with children and / or young people.

Risk management and positive risk taking.

Desirable Criteria

Working with vulnerable adults and / or children within a health and social care setting.

Involvement in multi-agency work.

Skills and Abilities:

Essential Criteria

Good interpersonal and communication skills.

Is empathetic and caring when working with people.

Good written and IT skills.

Desirable Criteria

Works within a scheme of delegated authority.

Works with complex situations and conflict and resolve people's complaints.

Assesses and record eligible and noneligible needs, drawing on evidence-based practice to inform your response.
