Care Outlook is seeking a highly organized Field Care Coordinator in East Sussex, supporting in Peacehaven and the surrounding areas. Core Duties & Responsibilities as Field Care Coordinator: Provide support in the day-to-day running of the office, including assisting with care coordination tasks. Spot checking carers in the community, identifying good practice, identifying and reporting training concerns back to the registered manager. Ability to deliver high standards of care. Participate in the paid on-call rota. Completing care assessments and writing up Care Plans. Building relationships with care staff and service users. Assisting the registered manager in preparing for inspections by the local authority and CQC. We offer the following: Salary £28,000 per annum plus On-Call additional payment. £300 Refer a Friend Bonus. Company issued mobile phone. Excellent Training for Care Certificate & ongoing professional development. 20 Days Holiday plus Bank Holidays. Opportunity to work in an expanding leading home care provider. We are looking for a Field Care Coordinator who is: 100% committed to helping improve the quality of life of vulnerable people. Experienced in providing care services (essential). Flexible to cover on-call to meet the needs of the business (essential). Knowledgeable of CQC standards and CQC Compliance (essential). Possessing a Full Driving Licence and access to a vehicle (essential). Experienced as Field Care Supervisor or 2 years working within Health & Social Care (preferred). Holding NVQ Level 3 Health & Social Care (preferred).
