Job summary Crays Primary Care Network are looking for an enthusiastic, forward-thinking Clinical Pharmacist to join their ever-growing team. The Crays Collaborative PCN is a network of 6 GP surgeries which are Broomwood Surgery, Crescent Surgery, Derry Downs Surgery, Gillmans Road Surgery, Poverest Medical Centre and St Mary Cray Practice. Together they cover a patient population of over 35,500 patients, most of whom live in deprivation. Two of their practices are training practices. They are a highly supportive PCN whose practices work closely together, understanding the importance of a good work-life balance with an open door ethos. The PCN caters to the healthcare needs of their patients by ensuring they make the best use of collective resources across practices, local healthcare providers and professional groups to allow greater resilience to primary care. To achieve this, they are supported by additional roles such as paramedics, Clinical Pharmacists, social prescribers, Health and wellbeing coach, nurses, care coordinators, mental health practitioners. You can expect to work across no more than 2 to 3 practices across the PCN. Occasionally group activities will take place at the wellbeing cafe. Main duties of the job Improve the physical mobility of patients who suffer from illness, injury, disability or aging Provide diagnoses, determine effective treatments and educate patients Evaluate patients' physical dysfunction, develop personalised care plans and administer therapeutic treatments Engage directly with patients assessing their physical conditions, diagnosing disorders and proposing treatment plans About us BGPA is a federation of all 43 Bromley practices working collaboratively to enhance the health and wellbeing of Bromley residents, covering over 350,000 patients, 100% of the Bromley population. BGPAs partner organisation is Bromley Education and Training Hub (BETH). BETH supports education, training and professional development across primary care in Bromley and beyond. BGPAs goals are to work strategically with Bromley practices to help secure the best services for patients whilst working together to support member practices in the challenges of a changing NHS. BGPA aims to improve the morale of general practice in Bromley by sharing expertise, services and supporting its workforce. BGPA will make a positive impact on medical services in Bromley by working closely with One Bromley Partners including NHS SEL ICB, local NHS trusts, local providers and patient groups, to improve the delivery of healthcare to the local population. Job responsibilities Perform skilled physiotherapeutic assessment of patients Use clinical reasoning skills to provide an accurate clinical diagnosis Refer to other medical and health care professionals Assess, develop and implement physiotherapy treatment for specialist caseload, and provide advice in specialist field Formulate, develop and deliver individualised treatment plans based upon evidence-based practice, manual assessment and appropriate clinical reasoning Communicate effectively with patients to ensure informed consent for assessment and treatment, providing the patient with a good understanding of their conditions and maximise their rehabilitation potential Keep abreast of evidenced based practice in the musculoskeletal and sports injury training area by use of relevant reading, attendance at in-service training, external courses and database searches Ensure good working knowledge of national and local standards and monitor quality as appropriate Have effective communication with all other PCN personnel Manage and prioritise own clinical caseload efficiently and effectively Provide clinical reports to supply clinical prognoses and comprehensive discharge planning Person Specification Experience Essential Experience within Primary Care Desirable Experience with the EMIS system Skills & Abilities Essential Excellent communication skills, both written and verbal Interpersonal skills to establish a rapport with patients and their families The ability to explain treatments simply to patients and their families Teamwork skills to collaborate with other healthcare professionals Problem-solving ability Organisation and administrative skills A firm but encouraging and empathetic attitude The ability to motivate others in order to get them to engage with their own care A genuine concern for the wellbeing and health of patients A real interest in anatomy and physiology The ability to work under pressure and manage your time effectively IT skills A flexible approach to work Qualifications Essential Member of the Chartered Society of Physiotherapists (MCSP) Member of the Health & Care Professions Council (HCPC) BSc (Hons) Physiotherapy (2:1 or above) Desirable PG Cert (or above) in relevant MSK field, working towards full MSc (Post Registration) Acupuncture Qualification(s) Injection therapy Qualification(s)
