The Surrey Continuing Healthcare Team is responsible for ensuring that patients referred for NHS Continuing Healthcare funding receive a high-quality assessment of need, ensuring that the patient is at the heart of the assessment, and that their views and those of their carers are taken into account within national timescales. Responsible for supporting the administration of the Referral process (including Fast Track referrals) and the CHC Coordinator, ensuring that the appropriate documentation has been provided and quality checked for accuracy. This includes arranging appointments for initial and review assessments for clients regarding their eligibility for NHS Continuing Healthcare and NHS-funded Nursing Care in a timely manner, and ensuring that all outcomes have been issued to referrers and clients/representatives. Discharge to assess allows patients across Surrey to be discharged freely out of hospitals to other care environments, helping to maintain hospital flow and reduce strain on the acute system. To act as a first point of contact for clients, their families and carers, the general public, acute and community providers, other health and social care staff, and other external agencies regarding referrals to facilitate the referrals process, logging all new referrals in a timely manner and prioritizing as agreed by CHC Management.
