We have an opportunity for an enthusiastic foundation level pharmacist to join our clinical pharmacy team to provide maternity leave cover for 12 months. This position is suitable for a recently registered pharmacist or a pharmacist with experience outside of secondary care. You will work with our friendly and supportive pharmacy team and will have the opportunity to develop a range of hospital pharmacy skills, working within the clinical pharmacy Homecare service, Medicines Information, and the Medicines Optimisation team to provide a clinical pharmacy service at ward level and to day case patients. Main duties of the job

You will work with the Homecare team to clinically validate prescriptions for patients receiving medicines via the Homecare service. This covers a range of clinical specialities, predominantly rheumatology, dermatology, and gastroenterology. You will work with the established pharmacy team to provide a clinical pharmacy service to the Medical Infusion Unit, through clinical validation of prescriptions and implementation of waste reduction strategies. You will work in Medicines Information to answer medicines-related enquiries from healthcare professionals within both secondary and primary care, and from patients recently discharged from our hospitals via the patient helpline. You will have an opportunity to work with the Medicines Optimisation team to provide a high-quality clinical pharmacy service to inpatients at ward level. Training and support will be provided which will enable the successful candidate to gain a thorough knowledge base and experience of clinical pharmacy. You will be supported through a structured induction process and will be provided with a range of training and development opportunities to build your foundation portfolio. The successful candidate will contribute to our dispensary late night and weekend service on a rotational basis. Job responsibilities

The post holder is accountable for his/her own professional actions but will always be working under the guidance of senior colleagues who will be available for reference if required. The post holder will be required to provide pharmaceutical care to patients in various specialities on a rotational basis. They are responsible for ensuring that the legal and statutory requirements in relation to medicines are met and for ensuring the delivery of pharmaceutical care to patients meets both national and local standards. On rotation the post holder will be required to: Carry out duties in the dispensary to help ensure that medicines are provided in a timely fashion for inpatients. These duties will include: Act as the Responsible Pharmacist in line with local procedure. Clinical and accuracy checking of prescriptions. Dispensing medicines. Dealing with medicines for clinical trial patients. Ensuring adherence to medicines legislation and accurate record keeping especially for those medicines covered by the Misuse of Drugs Act. Ensuring medicines are selected and employed in accordance with the Trust Medicines Policy and the Drugs Formulary. Supervising technicians and trainee pharmacists. Educating patients and/or their representatives about their medicines. Liaising with various members of the multi-disciplinary team. Inputting patient-related data into computers. Managing and prioritising own workload to tight deadlines and ensuring urgent work is done. Moving stock when necessary. Making straightforward decisions relating to medicines provision. Carry out duties in the Technical Services section, which will include: Clinical and accuracy checking of prescriptions. Dispensing and supervision of others in dispensing (by manipulation and reconstitution) injectable medicines (and ensuring safe and appropriate disposal of sharps and other waste). Preparation and dispensing of special formulations for external or internal use. Liaising with medical and nursing staff with a view to providing advice about intravenous nutrition (TPN) prescriptions. Handling and dispensing medicines used in the treatment of cancer (cytotoxic medicines). Person Specification

Experience

Experience in pharmacy practice obtained through one year’s pre-registration training and examination. Experience of working in a multidisciplinary team and experience in UK hospital pharmacy. Qualifications

Master's Degree in pharmacy obtained through four years undergraduate training. Membership of the General Pharmaceutical Council achieved by 12 months pre-registration training/experience and examination. Membership of the Royal Pharmaceutical Society of Great Britain. Membership of the United Kingdom Clinical Pharmacists Association. Membership of the Guild of Healthcare Pharmacists. Special Knowledge/Skills

Good written and verbal communication skills. The post holder must be equally at ease talking to patients, medical, and nursing staff of all grades in both primary and secondary care and will need to present written and verbal reports to senior hospital staff. The post holder must have basic keyboard skills and familiarity with routine office software word processing, spreadsheeting, and PowerPoint. Physical Attributes

Must be physically able to visit wards and departments within the hospital and to visit other Trust sites as required by the activities of the job. Exhibits an enthusiasm for development of the practice of Pharmacy. Able to work as an individual and within a team. Possesses personal organisational attributes. Responds positively to service deadlines. Able to plan and manage own workload. The post holder must display an understanding of and ability to deal with the confidential and sensitive information with which they will come into contact on a daily basis. The post holder will need the necessary skills to be able to communicate complicated medication issues to patients and carers in order that they may understand and make best use of their medicines. The post holder must display an understanding and ability to communicate and deal with all patients and/or carers some of whom may have language, sensory or learning difficulties, or who may be dying or distressed because of their diagnosis or state of health. Clean driving licence and access to a vehicle with the ability to travel independently in line with requirements. Disclosure and Barring Service Check
