Job Summary: Kingston and Richmond Foundation Trust is seeking a highly skilled and motivated Frailty Physiotherapist Team Lead to join our dynamic team. This pivotal role involves working across the Emergency Department, Acute Assessment Unit, Clinical Decisions Unit, and Frailty Unit. The successful candidate will work as an autonomous practitioner, providing specialized therapy assessments and treatments, and leading a team of physiotherapists to deliver evidence-based, best practice frailty care. Key Responsibilities: Conduct comprehensive assessments of patients with complex presentations and frailty syndrome. Formulate and implement individualized treatment plans, ensuring timely and appropriate discharge. Collaborate with multidisciplinary teams to coordinate patient care and support. Lead and manage the physiotherapy team within the Frailty department, including supervision, performance management, and development. Provide clinical supervision and support to junior physiotherapists, physiotherapy technicians, and students. Drive service development and audits within the Frailty team. Maintain accurate and up-to-date patient records and documentation. Participate in weekend working and contribute to the therapy flow rota. Communicate effectively with patients, carers, and the wider MDT to ensure coordinated care. Qualifications and Experience: Diploma/BSc Degree in Physiotherapy. State Registration with the Health and Care Professions Council (HCPC). Proven experience as a Senior Physiotherapist in Elderly Care, including rotations in Medical, ITU/Surgery, and Orthopaedics. Must have 6 months experience within the NHS within the last 3 years. Experience working in A&E, ED, AAU, CDU, and Frailty settings. Strong leadership and team management skills. Excellent communication and interpersonal skills. Commitment to continuous professional development and evidence-based practice.
