The post holder will provide clinical management for patients where preventing hospital admission is in the best interests of the patient and is possible. Examples include: acute illness, exacerbation of chronic medical conditions, frailty, end of life, and DVTs.
The post holder will work within a multidisciplinary team to deliver community care to patients, which includes administration, therapy, nursing, and matron colleagues.
Whilst it is not a legal requirement, Hertfordshire Community NHS Trust encourages and supports all staff to be vaccinated as this remains the best line of defence against COVID-19.
