Are you a dedicated General Medical Practitioner (GP) looking to make a real difference in the community? Hertfordshire Community NHS Trust is seeking passionate GPs to join our innovative, patient-centered teams on a fixed-term basis. To work as part of a multidisciplinary team undertaking remote clinical support through telephone and video consultations within our Hospital at Home service to patients across East & North Hertfordshire. Our Hospital at Home service provides short episodes of hospital level care to patients in their own home either as early supported discharge or to prevent an admission or hospital attendance which helps to release capacity in hospital. As an experienced GP you will work as part of a team within our Integrated Care Coordination Centre in Stevenage to deliver high quality medical support to patients looked after by HCT. The team which includes GPs, Advanced Care Practitioners, Pharmacists, Nurses and Therapists provides high quality innovative care to our patients enabling them to be cared for in their own home environment as part of an integrated system wide approach to Hospital at Home care. The role also provides the opportunity to further develop new pathways and implement the use of new technologies within our Hospital at Home service. Main duties of the job

The post holder will be required to: Have strong relationship building skills to work positively as part of a multi-disciplinary team and across partner organisations including but not limited to ambulance, out of hours providers, social care and mental health clinicians. Have excellent communications skills. Maximise the best clinical care delivered in the patient's own home to reduce the need for hospital admission. Assess patients via telephone, video call or jointly with other appropriate members of the team who are face to face with the patient with access to Point of Care Testing, diagnostic testing and dedicated support lines with acute consultants to support your decision making. Agree a clinical care plan with the support of remote monitoring technology where clinically appropriate to support the patient to remain in their home environment wherever possible including the ability to initiate IV antibiotics. Liaise with GPs and acute consultants to ensure the patient receives the most appropriate care. Participate in regular specialist MDTs. Utilise the community nursing and therapy clinical assessments, observation and examination findings, including those on the remote monitoring dashboard, and then review the patient, make a diagnosis and a management plan and order tests as required. The post holder will provide clinical management for patients where preventing hospital admission is in the best interests of the patient and is possible. Examples include; acute illness, exacerbation of chronic medical conditions, frailty, end of life and DVTs. The post holder will work within a multidisciplinary team to deliver community care to patients-this includes administration, therapy, nursing and matron colleagues. Person Specification

Education/Professional Registration

GMC full registration with a license to practice Included on the National Performers' list as a general medical practitioner MRCGP, or other recognised postgraduate qualification Work Experience

Experience of working as a GP delivering general medical services in the NHS, or other healthcare systems Experience of improving clinical services Excellent oral and written communication skills Good organisational skills Ability to travel in order to meet the requirements of the post and cover units where required Openness, flexibility and clarity of independent clinical decision making Experience of using systmOne Teaching and Training
