In partnership with GP colleagues, the Trust MDT, and consultant physicians, you will be responsible for the care of patients in one or more community hospitals. You may be asked to provide medical care on site, virtually, or by phone at any of the community hospitals in emergencies temporarily or long term.
These posts are for a fixed 12-month term with both for up to 40 hours per week. Sessions will work according to a Job Plan. Shifts will be 4 or 8 hours in length and cover the hours of 08:00 to 18:30 Monday to Friday. Phone advice for community hospitals from 08:00 to 18:30 is shared by the medical team. Those wishing to work less than full time are also encouraged to apply.
There is the expectation of working a 1 in 4 weekend shifts on a rota at Worcester City Inpatient Unit, with additional payment at GP locum rates, but there is no overnight on-call commitment.
Main Duties of the Job You will work alongside a larger integrated neighbourhood team providing pathways from community to inpatient and back to community care in a seamless model, supporting purposeful admission from both the community and post-acute inpatient care.
The main purpose of the post is to provide generalist medical care for patients undergoing rehabilitation in an inpatient environment. Most of these patients are frail, elderly, with multiple comorbidities, some of whom may develop intercurrent problems such as infection, falls, and delirium requiring subacute care.

Experience of working as a General Practitioner or as a specialty doctor in an environment treating frail, elderly patients
Qualifications
Primary Medical Qualification
Entry on the GMC Register
Diploma in Geriatric Medicine
MRCGP (or equivalent to gain entry to the national GP performers list if applicable)
Knowledge
Detailed knowledge of the management of patients with complex medical morbidities including stroke and frailty
