An exciting opportunity has arisen to join the NHS Wales award winning Stay Well @ Home Team at the Royal Glamorgan hospital. We are looking for a skilled, organised and enthusiastic individual to join our established and friendly, integrated team of Occupational Therapists, Physiotherapists, Social Workers and Assistant Therapy Practitioners (ATP's). Main duties of the job

Key aspects of the role include working as part of a multi-disciplinary team to undertake specific clinical/therapy based interventions, administrative tasks and liaison with local authority and third sector based community services to promote and support timely discharges from the Accident and Emergency (A&E) and the Acute Medical Unit as appropriately delegated by the registered clinicians. As part of the team, the ATP plays a vital role in providing high quality care and interventions for our patients with the service aimed at reducing length of stay, improving admission avoidance and reducing avoidable readmissions through partnership working with community based services. Whilst this post is predominantly hospital based, there will be opportunity to undertake community based visits where required. You will be able to support the assessment and discharge planning of people with a range of functional difficulties within the bounds of the ATP role. Patient management will include responsibility for the delivery of care including liaison with patients, family, members of the MDT and external agencies. You will also need to have excellent time management and prioritisation skills and the ability to communicate effectively both verbally and in written form. Qualifications

Evidence of good education, to include English, Mathematics GCSE or possess equivalent qualification, or skills knowledge and experience OCN/Agored accredited modules or relevant NVQ level 3 Knowledge relating to a range of core OT/PT activities Knowledge of the discharge planning processes, specific conditions and forms of therapy Successful completion of relevant competency based training Experience

Experience of developing/delivering education advice to individuals/groups Experience of multi-disciplinary team working Experience of working as HCSW in a medical or rehabilitation setting Skills

Excellent verbal and written communication skills Excellent ICT, computer literacy and keyboard skills The ability to work independently whilst under appropriate supervision Ability to work with patients with communication barriers/motivational issues Personal Qualities

Ability to work independently and as part of a team Ability to motivate self and others Confident in approaching tasks Disclosure and Barring Service Check
