We are looking for a Practice Nurse experienced in long term conditions to join our nursing team.
Ideally you will work between 20 and 32 hours a week, between the hours of 8:00am and 6:00pm, Monday to Friday.

RGN diploma/degree NMC registration Experience
Experience of working within a GP practice Experience of long term conditions management Excellent organisational skills and ability to prioritise work. Ability to maintain strict confidentiality Ability to work flexibly as part of a team and independently Knowledge of SystmOne clinical system Experience of learning disabilities management Disclosure and Barring Service Check
