Mid Yorkshire Hospitals Trust, North Kirklees and Wakefield Councils are working collaboratively with Yorkshire Cancer Research and Sheffield Hallam University to implement the Active Together model to individuals diagnosed with cancer in Wakefield and North Kirklees. The Active Together programme will run alongside the existing diagnostic and treatment pathways to ensure patients are optimally prepared for treatment and beyond. It aims to maximise resilience to treatment, improve longer term health, minimise side effects and empower patients to be partners in their healthcare. This is a funded 5 year programme which will have staged implementation into all relevant cancer pathways. Patients will receive individual assessments and be triaged into a support pathway suitable to their needs. This may be delivered in Mid Yorkshire Hospitals, selected council facilities closest to the patient’s home or may be a self-management pathway. Wakefield Councils are looking to recruit into the role of Activator to support delivery within this programme. This role will work with the Specialist Help Improvement Officer who is leading on the delivery of this scheme on behalf of Wakefield Council and will be based mainly across 2 of the 6 leisure centres across Wakefield District, Sun Lane Leisure and Aspire at The Park. The role will be providing consultation and exercise on a 1-2-1 and group basis, to patients who are pre and post Cancer treatment and have been assessed as requiring this service through initial triage through NHS Mid Yorks. The ideal candidate to support this work must be able to plan and deliver all touch points within the programme. The successful candidate must possess excellent interpersonal, consultation, written and verbal communication skills, be able to work effectively with and gain the trust of a wide range of stakeholders (internal and external partners) whilst ensuring they are committed to excellent service delivery, ensuring that prescribed physical activity is suitable for the needs of the participant. Prior knowledge and experience of working with patients on Health Referral Pathways, specifically those who have been pre or post cancer treatment is beneficial and prospective candidates must hold a minimum recognised Level 3 Exercise on Referral qualification. There will be an expectation to complete and achieve a Level 4 Cancer Rehabilitation qualification once in post if this is not already held. Want to Know More?
