Job summary We are excited to recruit to the post of Health and Wellbeing Coach. This role will work within our Primary Care Network providing 1:1 personalised support to people who are referred to our multi-disciplinary Wellbeing Team. The successful candidate will have good communication, negotiation and people-management skills and act with compassion and integrity. They will have experience of using coaching approaches, frameworks and models or other helping strategies, for example, motivational interviewing. This is a vital role within the Primary Care Network and the post holder will provide health and wellbeing coaching for the most vulnerable people in the community, including the frail/elderly and those with other long-term conditions. They will work closely with the patient to understand their individual needs and motivation levels, building a plan and working towards their personalised identified outcomes. The successful applicant will be primarily based at Ledbury Health Partnership and the role will include patient work at a variety of locations in the community. Some home working may be considered for this role. The successful candidate will also support and may be required to work from other GP Practices based in East Herefordshire Primary Care Network (PCN) including Nunwell Surgery, Colwall Surgery, and Cradley Surgery. * This position is for 25 hours per week * * Interviews are expected to be held W/C 24th February 2025 * Main duties of the job This is an exciting opportunity to work with GP practices to help people who need support to develop the knowledge, skills and confidence to manage their own health and wellbeing. Specifically the role will support patients with long term health conditions and/or lifestyle challenges which affect their wellbeing. They will work closely with GP and practice teams to self-manage a caseload, ensuring appropriate support is available to people, families and carers. The successful candidate will work alongside social prescribing link workers and care coordinators to provide an all-encompassing approach to personalised care and will enable people to work out how best to use the health and care system. The Health and Wellbeing Coach will work closely with the practice teams to ensure their role is understood within practices, reporting back on patients' progress to the relevant clinicians and being part of multi-disciplinary team meetings. The role will also include ensuring patient activity and data is accurately recorded on the relevant systems and patient confidentiality is maintained at all times. Job Responsibilities

Main Responsibilities: Coach and motivate patients through multiple sessions to identify their needs, set goals, and implement a personalised health and care plan. Provide support to individuals, families, and carers to ensure they are active participants in their own healthcare; empowering them to take more control in managing their own health and wellbeing, to live independently, and improve their health outcomes through: Providing interventions such as self-management education and peer support Supporting people to establish and attain goals based on what is important to them. Working with social prescribers and Talk Community teams to connect to community-based activities which support their health and wellbeing. Provide support to local community groups and work with other health, social care, and voluntary sector providers to support the patients' health and well-being holistically. Ensure that fellow PCN staff are made aware of health coaching and social prescribing services and support colleagues to improve their skills and understanding of personalised care, behavioural approaches, and ensuring consistency in the follow up of people's goals where an MDT is involved. Raise awareness within the PCN of shared decision making and decision support tools and supporting people in shared decision-making conversations. Work with people with lower activation to understand their level of knowledge, skills, and confidence (their Activation level) when engaging with their health and wellbeing. Explore and support access to a personal health budget, where appropriate, for their care and support. Utilise existing IT and MDT channels to screen patients, with an aim to identify those that would benefit from health coaching. Knowledge & Skills Essential Knowledge of the personalised care approach and primary care networks. Understanding of the determinants of health to include social, economic, and environmental factors. Knowledge of public health issues and health inequalities in Herefordshire. Understanding of, and commitment to, equality, diversity, and inclusion. Knowledge of IT systems to include the ability to use word processing skills, emails, and the internet. Desirable Knowledge of the needs of patients with long term conditions. Knowledge of community development approaches and local programmes of work, Talk Community. Qualifications Essential Educated to GCSE or equivalent. Demonstrable commitment to professional and personal development. Desirable NVQ 3 or equivalent and/or relevant basic/first level professional qualification or working towards this. NHSE Approved training in motivational coaching, behavioural change, and goal setting. NB: Completion of the training will be an essential requirement for new Health & Wellbeing Coaches. Personal Qualities Essential Excellent communication and interpersonal skills. Ability to build rapport and establish one-to-one relationships whilst maintaining appropriate professional boundaries. Ability to effectively manage a variable workload. Willingness to participate in shaping the future of the provision by working with colleagues across the network and wider community. Experience Essential Experience of data collection and using tools to measure the impact of services, Patient Activation Measure. Experience in health promotion and health improvement. Desirable Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement - can include unpaid work. Experience of supporting people, their families, and carers to change health behaviours and improve wellbeing - can include unpaid work. Experience of supporting people with their mental health, either in a paid, unpaid or informal capacity. Experience of partnership/collaborative working and of building relationships across a variety of organisations including the voluntary sector. Experience using clinical systems - EMIS.
