UCRT is a community admission avoidance team available 7 days a week that will help patients stay at home when they are feeling unwell rather than be transferred to hospital. The UCRT team provides specialist nurse and occupational therapy led care in patients' own homes, visiting patients within two hours of receiving a referral to stabilise their immediate health needs and plan ongoing care and treatment as appropriate. The Band 3 Health Care Assistant will work as part of the UCRT Team to deliver acute care in patients’ homes, including the monitoring of observations, venepuncture, and supporting patients with personal care during an episode of acute ill health. Working for Our Organisation

To undertake through each patient’s individual care plan, those duties delegated by the Team Leader and Case Manager, which the Health Care Assistant has been trained and is competent and confident to perform. These tasks include: Personal care Monitoring of baseline physical observations Blood glucose monitoring Urinalysis Cannulation Wound care management Administration of nebulisers and medication Collection of specimens Administration of insulin (on completion of competencies).
