Job Overview UCRT is a community admission avoidance team available 7 days a week that will help patients stay at home when they are feeling unwell rather than be transferred to hospital. The UCRT team provides specialist nurse and occupational therapy led care in patients' own homes. They visit patients within two hours of receiving a referral to stabilise their immediate health needs and plan ongoing care and treatment as appropriate. Main duties of the job The Band 3 Health Care Assistant will work as part of the UCRT Team to deliver acute care in patients’ homes, including the monitoring of observations, venepuncture and supporting patients with personal care during an episode of acute ill health. UCRT is a multi-disciplinary team focused on delivering excellent care to patients in their own home to avoid admission to hospital. The team work well together and support one another.
