Provide is looking for dynamic, enthusiastic, and motivated Healthcare Support Workers to join us in our exciting Mid Essex Frailty Virtual Ward. The ward supports patients who are recovering from an illness, and this involves providing clinical interventions, personal care, and supporting the recovery and rehabilitation of patients within their own home. Healthcare Support Workers contribute to the assessment and treatment of individual patients on the caseload and communicate any changes in health and care needs to the registered staff. The post holder will require a valid driving license and access to a car. Main duties of the job

To visit patients within a community setting. To deliver a range of interventions as outlined in the care plan which will include supporting patients with personal activities of daily living, domestic activities, rehabilitation plans, and delegated health care tasks including a set of observations, basic wound care, and taking bloods. To gain informed consent and deliver sensitive, high-quality patient care. To encourage patients and carers in an active approach to achieving their goals and regaining independence. Supporting patients in regaining optimum mobility. To assist in identifying equipment and adaptations required in a patient's own home. To monitor patient progress and report back to the registered staff. To complete clinical and non-clinical records ensuring they are accurate, timely, and in accordance with the organisational, professional, and departmental standards and procedures. Maintain and develop knowledge, skills, and competence to ensure patients' individual needs are met in accordance with the band 3 competency framework. About us

Qualifications

NVQ 3 or equivalent including experience. GCSE in English and Maths. Previous experience in a healthcare setting. Experience

Understands professional conduct. Relevant experience to job role. Experience working in a community setting. Previous experience of lone working. High standard of written English & verbal communication. Work effectively as part of a team and alone. Ability to organise & prioritise own workload under supervision. Flexible, punctual & reliable. Circumstances

Ability to do shift work including weekends & bank holidays. Valid driving licence & own transport. Disclosure and Barring Service Check
