To deliver a range of interventions as outlined in the care plan which will include supporting patients with personal activities of daily living, domestic activities, rehabilitation plans, and delegated health care tasks including a set of observations, basic wound care, and taking bloods. To gain informed consent and deliver sensitive, high-quality patient care. To encourage patients and carers in an active approach to achieving their goals and regaining independence. Supporting patients in regaining optimum mobility. To assist in identifying equipment and adaptations required in a patient's own home. To monitor patient progress and report back to the registered staff.
