Health Care Support Worker, Living Well Centre Job summary The Living Well Centre is an established community drop-in centre situated adjacent to Rowans Hospice providing information, support, therapies and group activities for anyone living with a life-limiting illness in the community, carers support, and those that have been bereaved. We now have an exciting opportunity for an experienced Health Care Support Worker to join the existing team. The ideal candidate will hold an NVQ level 3 in Health and Social Care and possess excellent communication and interpersonal skills. NHS pension will be retained if transferring over from the NHS. Awareness of palliative care principles and philosophy within day care. To report all enquiries/conversations with patients/relatives and visitors to the staff/senior staff nurse or Manager/Deputy Manager of LWC. To record and pass on all messages received by telephone to the appropriate person staff/senior staff nurse or Manager/Deputy Manager of LWC. To establish and maintain effective communication and working relationships with multi-disciplinary teams within the Hospice and in the community. To communicate effectively with patients, carers and the bereaved to establish and maintain trusting relationships. To offer emotional support to patients and/or carers, recognising own limitations. To contribute to bereavement care offered to families by reporting any concerns related to risk assessment to the staff/senior staff nurse or Manager/Deputy Manager of LWC. About us

Job responsibilities

Key Areas of Responsibility/Key Accountabilities In conjunction with the Living Well Manager, offer a 5-day a week service, from Monday to Friday. To support the growing need for those who may look to Rowans Hospice Charity for help and support before they reach the final stages of their life. To support a facility where there are opportunities to inform, support, care and re-able people, their families and friends, who are affected by the diagnosis of a life-shortening or incurable illness or indeed to those who are approaching the end of their natural life span. To support an environment that provides companionship for both the patient and carer. To maintain the health and well-being of the local community as far as this may be possible. To liaise with members of the Clinical Team and Voluntary Support as directed by the staff/senior staff nurse or Manager/Deputy Manager of LWC. To promote a therapeutic environment utilising the skills of therapists/artists working within the setting and taking an active part in special projects, i.e., Work/Mindfulness. To report to the staff/senior staff nurse or Manager/Deputy Manager of LWC any relevant information on the condition of specific patients, their care or treatment. To report to the staff/senior staff nurse or Manager/Deputy Manager of LWC, Director of Clinical Services any matter affecting the smooth running of the unit. To report any complaints/safeguarding concerns from patients, their relatives, or others acting on their behalf to the Deputy Manager/LWC Manager/Director of Clinical Services. To ensure knowledge about patients/carers is maintained in the strictest confidence and is not divulged, except to professionals where it would materially assist with the programme of care or protection of the individual (See Staff Handbook Confidentiality/Data Protection Act). GDRP. To demonstrate a consistent level of courtesy and consideration to patients, their relatives and visitors. To promote a high quality of care by adhering to set standards. Support the trained nurse with the procedure for opening and closing the centre. Person Specification

Qualifications

Basic nursing care skills. Good communication and interpersonal skills. Basic IT skills and cyber security. Ability to work within a large team. Ability to use own initiative. Ability to respond to changing situations. Awareness of own coping strategies. Personal grief resolved sufficiently to work within a palliative care setting. Knowledge

Awareness of palliative care principles and philosophy within day care. Excellent interpersonal skills. Good written and verbal communication skills with the ability to communicate diplomatically. Good report writing and presentation skills with a high level of computer literacy. Experience working as a Healthcare Support Worker in other settings, preferably Hospital. Disclosure and Barring Service Check
