The post holder will work at all times under the delegation of a registered practitioner in the delivery of person centred care. The post holder will be responsible for following care plans and recording all personally generated observations and documenting care given in individual’s notes. The post holder will assist in the delivery of therapeutic interventions and activities to enhance the patients' rehabilitation and recovery and discharge pathway. The post holder will be expected to undertake flexible working hours, inclusive of unsocial hours and weekends.

Assist the Registered Practitioner in the assessment, planning, treatment, management and evaluation of care. Escorting the patient into the community following their care plan and rehabilitation and recovery needs. Assists patients to achieve physical and emotional wellbeing and comfort, encouraging independence where appropriate. Supports patients to access the local community via public transport and Health Board vehicles when appropriate. Respects the privacy, dignity, needs, beliefs, choices and preferences of patients and carers.
