The Healthcare Support Worker role may be assigned to work within our Hospice at Home Team or our Inpatient Unit at the Hospice, providing vital support to patients and their families during challenging times.
Under the direction of the Hospice Head of Hospice Home, Healthcare Support Workers will assist in delivering high-quality, person-centered care and support to patients with life-limiting illnesses, ensuring their comfort, dignity, and well-being.
Rota weekends bank holidays, may need to do nights (2 or 3 x 12.5 hour shifts)
Main duties of the job
Provide compassionate and empathetic care and support to patients with life-limiting illnesses and their families, respecting their dignity and cultural preferences. Assist patients with personal care tasks ensuring their physical comfort and well-being. Support patients with mobility, transfers, and positioning, using safe manual handling techniques and equipment. Facilitate late discharge and support overnight admissions into hospice care. Document patient observations, care interventions, and other relevant information in accordance with hospice policies and procedures. Assist patients with activities of daily living, including meal prep, serving, and feeding, ensuring their nutritional needs are met and dietary preferences are respected. Support patients in engaging in activities, hobbies, and social interactions to promote their well-being and quality of life. Maintain a clean, safe, and comfortable environment for patients, ensuring their rooms and communal areas are tidy and free from hazards. Provide emotional support and companionship to patients and their families, actively listening to their concerns and offering reassurance and empathy. Collaborate effectively with multidisciplinary team members to ensure coordinated and holistic care for patients and families. Participate in team meetings and care planning sessions, contributing insights and observations to inform patient care. Provide supervision and guidance working with the Volunteer Manager when necessary. About us

Assist patients with activities of daily living, including meal preparation, serving, and feeding, ensuring their nutritional needs are met and dietary preferences are respected. Support patients in engaging in recreational activities, hobbies, and social interactions to promote their well-being and quality of life. Maintain a clean, safe, and comfortable environment for patients, ensuring their rooms and communal areas are tidy, well-stocked, and free from hazards. Provide emotional support and companionship to patients and their families, actively listening to their concerns and offering reassurance and empathy. Collaborate effectively with multidisciplinary team members, including nurses, doctors, social workers, and therapists, to ensure coordinated and holistic care for patients and families. Experience
Previous experience working in a healthcare or social care setting. Knowledge of and adherence to relevant policies, procedures, and regulations governing healthcare delivery and patient safety. Experience working in a palliative or end-of-life care environment. Experience working with SystmOne. Experience working with Volunteers. Skills and Abilities
Strong communication and interpersonal skills, with the ability to build rapport and establish trusting relationships with patients and families. Excellent organisational and multitasking abilities, with attention to detail and the ability to prioritise tasks effectively. Compassionate, patient-centered approach to care, with a genuine commitment to supporting individuals and families facing life-limiting illnesses. Proficient in Microsoft systems.
