An exciting and rare opportunity has arisen to join a busy and passionate CMHT based within the Centre of Swansea as a Band 3 Healthcare Support Worker. The post holder will be required to work as part of a team providing support to patients in receipt of Secondary Care Mental Health Services as defined by Mental Health Measure 2010. This will involve assisting individuals to achieve and maintain optimum levels of independence by providing support, choice and dignity in their daily living via assessment, implementation and evaluation of agreed plans of care. Post holders will have the opportunity to engage with a caseload with both direct and indirect supervision from registered staff and engage in lone working. Main duties of the job

An element of the role will be supporting the qualified staff in facilitating a range of clinics such as Clozapine, Lithium, ADHD and physical health monitoring clinics consisting of taking observations and monitoring of medication side effects and it is desirable for the post holder to have phlebotomy skills. The post will also involve some administration work in relation to booking the clinics. Job responsibilities

There is the opportunity of working collaboratively with the Multi-disciplinary team (MDT) to ensure the maximum benefits to patients/residents using a patient centred approach to care which is often extremely rewarding. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply. Person Specification

Qualifications

Minimum of Qualification Credit Framework - QCF Level 3 (formerly NVQ Level 3) in Health or willingness to complete in agreed timeframe Evidence attendance at courses / study days pertinent to speciality Evidence of competence assessment in administration of Buccal Midazolam Knowledge

Evidence of maintaining their personal development Demonstrates an understanding of the scope of the role in the context of the nursing team Knowledge and understanding of the Code of Conduct for Healthcare Support Workers in Wales Knowledge of the fundamentals aspects of care and the importance of the promotion of health and wellbeing Understanding of basic physiology, e.g. normal vital signs, fluid balance, nutritional requirements Understands the principles of confidentiality Understands the principles of consent prior to undertaking care Understands the principles of Safeguarding adults/children to keep patients within their care safe Understanding of when to seek advice and escalate to the appropriate registered professional for expert help and advice Understanding of the Health Board's Values and Behaviour Framework Understanding of caring for individuals with particular conditions; for example dementia, mental illness, learning disabilities relevant to area of practice Experience

Significant experience of working as a Healthcare Assistant Previous relevant experience in specialty Aptitude and Abilities

Caring, compassionate attitude to patients and clients Able to communicate effectively with patients/relatives and carers and all members of the multidisciplinary team Able to undertake specific procedures involving physical skills commensurate with practice area e.g. recording of vital signs, venepuncture Ability to work effectively as a team player under appropriate supervision, and as part of a multi-disciplinary team. Able to organise self and others to achieve delegated task Able to effectively and appropriately escalate concerns to reduce risk and promote patient safety. Ability to deal with non-routine and unpredictable nature of the workload, prioritise workload and individual patient contact Customer care skills Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. Lead Nurse - Community Mental Health Services £24,433 to £26,060 a year per annum (pro rata if part time)
