The Urgent and Intermediate Care Team is pleased to be offering these posts to join our Care at Home Team, providing patient-centred care to individuals in the community, alongside community nurses and allied health professionals.

We are looking for passionate HCSWs to join our Pembrokeshire Team to support patients to stay in their own home.

You will be working in the Community but have a contractual base at either South Pembrokeshire Hospital or Withybush General Hospital. The service operates 7 days a week between 07:30 – 22:00 and staff shifts are within these hours. You will be working in patients’ homes across the county, and the ability to travel to locations in a timely manner is essential.

In this varied and exciting role, you will be providing personal care to support patients in their own home. This can be for patients waiting for long-term care, reablement and rehabilitation, continuing health care, or as part of their end of life/palliative care plan.

The post holder will be required to support colleagues in delivering a high standard of person-centred care in accordance with Care and Treatment Plans.

Excellent interpersonal and communication skills are essential, as is the ability to work autonomously and with colleagues.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
