An exciting opportunity has arisen to join the Gedling LMHT. The LMHT provides specialised mental health assessments and evidence-based treatment within the Gedling area. The service is committed to the principles of recovery; aiming for people to be empowered to be as independent as possible and wellness recovery action planning.
A full UK driving licence and vehicle for business use is required for this post; however, reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
Gedling Local Mental Health Team are looking for an enthusiastic, dynamic and motivated Band 4 Health Improvement Worker to work across the Gedling area.
This role will involve working alongside mental health teams and general practices to proactively identify and help the patient to manage cardio-metabolic risk factors associated with serious mental illness. You will coordinate and communicate between the different parts of the service that the patients are in contact with.
People with a Serious Mental Illness die on average 15 – 20 years earlier than the general population often due to the under-identification or poor management of their cardio-metabolic risk factors.
