To support the PCN Manager to plan the inequalities programme for the PCN Direct Enhanced Service (DES) or equivalent and additional initiatives. To lead the delivery of the agreed DES programme, supporting the PCN Manager to lead the care coordinator and social prescriber teams to ensure safe and effective care and support to reduce inequalities delivered to the population of Anfield and Everton PCN.

To plan and deliver inequalities and associated patient and staff events. To represent the PCN at meetings relevant to the inequalities and associated agendas, and report progress to the PCN Manager.

To develop reports about the work undertaken and support their analysis. To work closely with the PCN Administrator to ensure effective communication of inequalities work planned and undertaken. To supervise the care coordinators and social prescribers and support the PCN Manager to create and monitor their workplans and development plans. To provide project management support to the PCN Manager, working on defined projects, including monthly reporting and delivery of quality improvement initiatives. To demonstrate effective leadership and communication skills, whilst working within relevant legal and ethical frameworks and in accordance with organisational protocols and policies.

Collaborative working relationships:

Work within the ARRS Team and across the 5 practices, leading and evaluating inequalities initiatives to promote quality improvement activity. Collaborate with members of the PCN, patients, and their carers when managing and coordinating care. Work in partnership with members of the PCN workforce including medical, nursing, pharmacy, other care coordinators, link workers, and health coaches to personalise care. Work with external stakeholders and organisations.

Management:

Use resources effectively to manage delivery in line with national and local guidance, and make recommendations for change where improvements can be made. Follow professional and organisational policies.

Education, learning and development:

Comply with all organisational and statutory requirements (health and safety, infection control, equality and diversity, confidentiality, safeguarding adults and children, information governance). Engage in developing objectives to inform Personal Development Plans. Participate in the upskilling of all relevant staff. Support practice staff and respond to requests for advice and assistance in respect of inequalities initiatives.

Complete all mandatory and statutory training required by the role. Take responsibility for personal development, learning, and performance and maintain education through attendance at any courses and/or study days necessary to ensure that professional development requirements are met. Undertake additional training where necessary to provide enhanced services and participate in training programmes implemented by the PCN/practices as part of this employment. Understand and demonstrate the characteristics of a role model to members in the team and/or service.

Quality:

Under supervision and support of the PCN Manager, the post-holder will strive to maintain the highest quality of delivery within the PCN, and will: Participate in clinical governance activity and contribute to the improvement in quality of health outcomes through audit, risk management and Quality Improvement. Alert other team members to concerns about risk, quality, and safety. Participate in investigation of incidents and events as required. Collect data for audit purposes and support their analysis. Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the team performance. Work effectively with individuals in external organisations to meet strategic, operational and patient needs. Effectively manage own time, workload, and resources.
