Health Management (CDM) Nurse - General Practice
Stirling Lakes Medical Centre and Kingsley Woodvale Medical Centre Casual Role - 13 hours per week (across both centres) Health Management (Chronic Disease Management) Registered Nurse or Enrolled Nurse About the role Deliver quality clinical nursing services to Doctors and patients including chronic disease management, triage, vaccinations, wound care, pathology collection, health assessments, recall, coordination of the treatment room, cold chain management and sterilisation.
