The Dementia & Older People's Mental Health Service in Cornwall Partnership NHS Foundation provides assessment and treatment to people living with dementia and adults with functional mental health difficulties and increased complexity associated with physical ill health and the physiology of ageing. This post is for a Community Mental Health Healthcare Assistant working with people who have a dementia illness and/or a functional mental health illness, the latter the age of 75 and over. The post is based within the West Locality of Cornwall, within Coombe Team, visiting people at home, in care homes and supporting our community health and social care teams. The team in which you would be based has the following disciplines: A Consultant Psychiatrist, a Team Manager, a Memory Assessment Nurse, Band 6 CPN's, a Clinical Psychologist, Primary Care & Dementia Practitioners, a Dementia Liaison Nurse, fellow Health Care Assistants, and an Occupational Therapist. You will support the Community Mental Health Nurses by visiting patients on their caseloads. Experience of working with people with dementia and/or mental health disorders is desirable for the role. The ability to work in partnership with carers/family members and services in the community is essential. Main duties of the job

The post holder will work closely with our qualified staff to deliver specific targeted interventions for people living with dementia and frail adults living with a range of mental health needs with the increased complexity associated with physical ill health and the physiology of ageing. Qualifications

NVQ Level 3 qualification in care or equivalent experience A basic understanding of current National Guidelines and Mental Health Act/Mental Health Capacity Act requirements Experience

Experience working with individuals living with dementia Previous knowledge and experience working with individuals with complex mental health or social needs Experience in community-based roles or visiting service users in their homes Disclosure and Barring Service Check
