An opportunity has arisen for a motivated, enthusiastic Band 7 Registered Nurse within the Community Heart Failure Service. The successful candidate will work in a variety of health care settings; GP Clusters, different hospital & community sites, Primary Care & patients' home to deliver specialist heart failure care to people with heart failure, across the Health Board. The Service operates in a dynamic, challenging environment with learning opportunities to learn & work with multi-disciplinary colleagues passionate about providing evidenced based best practice to our patient group. You will need to work autonomously and as part of a team, with excellent communication skills & demonstrate the Health Board Values. You will need the ability to make advanced clinical decisions in a safe & competent manner. The role requires a flexible approach to work, with commitment and ability to prioritise your workload. You will work with health care professionals across care boundaries and closely with the Clinical Nurse Specialists, General Practitioners & Consultant Cardiologists. You will have access to study/training opportunities to complement the clinical environment and enhance your skills. You will require leadership skills & ability to manage staff, undertaking line management duties when required. You will be able to share your heart failure expertise through teaching colleagues as well as providing patient/carer education. Main duties of the job

Responsible for the direct delivery of clinical care & be clinically competent to act as an independent clinician for heart failure patients. This will involve autonomous clinical decision-making, planning highly specialist programmes of care. Independently manage patients with heart failure & refer on to primary & secondary care colleagues including Virtual Wards. To provide expert clinical information, support & advice to patients, carer's & the wider multidisciplinary team to promote streamlined, effective, patient focused care. The post holder will be responsible for undertaking specific interventions & procedures, safely & effectively as part of the MDT. To deliver heart failure clinics in the community setting, including home visits and/ or telephone follow-up. Maintain highly developed clinical skills, which require accuracy e.g. undertaking phlebotomy & ECG's with interpretation. Work towards training as a non-medical Independent Prescriber; in meantime, recommend the prescription of evidence-based medicines within appropriate scope of professional practice supported by the heart failure Lead Specialist Nurse & HF Cardiology MDT. As a qualified non-medical Independent Prescriber, recommend and/or recommend the prescription of evidence-based medicines within appropriate scope of practice supported by DSMP, adhering to SBUHB prescribing policy and guidelines. Qualifications

Registered Adult Nurse and current NMC Registration Level 6/7 Qualification in Heart Failure or relevant experience Evidence of CPD in heart failure Experience

Substantial experience within heart failure care/management/Cardiology Experience as a Band 6 CNS Evidence of Undertaking/ Understanding/Interpretation of ECG's Previous experience within Primary & Community Care Experience managing own caseload Experience in teaching/leadership roles Disclosure and Barring Service Check
