We have a very exciting opportunity to join our integrated Heart Failure Service at Calderdale and Huddersfield NHS Trust. We are seeking a highly motivated individual with excellent interpersonal and communication skills. The successful candidate will be responsible for the development of a cohesive management programme for patients with heart failure in line with local and national guidelines, working across acute, community and virtual ward settings. The service will operate 7 days a week, with a view to extending hours of 8am to 8pm in the future and requires flexible working within an evolving proactive, dynamic, and committed multidisciplinary Heart Failure Team. The aim of the role is to work autonomously, within the team, managing own caseload and to identify patients who can be supported on the Heart Failure Virtual Ward, reducing length of stay, preventing unnecessary admissions to hospital, and improving heart failure management and control with optimisation of evidence-based treatments. Job responsibilities

Will work across the hospital with responsibility for patients with heart failure in the inpatient, outpatient, community and virtual ward setting. Will be responsible for the early identification of patients for the Heart Failure virtual ward from the acute setting and supporting the ongoing care for patients who are actively being managed on the virtual wards. The post holder will take overall responsibility to develop the specialist area of practice in partnership with other members of the specialty team (Lead clinician, Matron, service Lead etc) and will work collaboratively to develop services and quality of care delivered. The post holder will be expected to act as the patients advocate at all times. The post holder will carry responsibility for their defined caseloads and offer highly specialist service advice which includes setting the standards of care, the assessment of care needs and the development, implementation and evaluation of programmes of care. The post holder will contribute to achieving the objectives of the role and work within the clinical governance framework. Will work autonomously managing own clinical caseload and to support the virtual wards. Will use relevant specialist training, knowledge, and expertise to clinically diagnose, assess and deliver evidence-based care through education and advise on self-management. Person Specification

QUALIFICATIONS / TRAINING

Post registration experience/qualification relevant to the specialism or willing to work towards. Non-medical prescriber or willingness to undertake this. Teaching qualification. KNOWLEDGE, EXPERIENCE & EXPERTISE

Understanding and appreciation of diversity. Knowledge of Healthcare IT systems. Demonstrate advanced clinical skills in cardiac nursing with reference to heart failure. Awareness of clinical governance. Manage complex issues and environments. Commitment to develop self and others. Ability to work flexibly according to service need. Ability to adapt to change and innovation. Awareness of CHFT values and vision. Willingness to further develop specific skills as desired by the service. Ability to think logically, use initiative and problem solve. Ability to work independently and as part of a team. Venepuncture, cannulation and IV skills. Experience in end-of-life care. Community experience. COMMUNICATION AND RELATIONSHIPS (INCLUDING MANAGEMENT RESPONSIBILITIES)

High level of communication skills. Engagement of patients and carers to support self-care strategies. Experience of lone working. Disclosure and Barring Service Check

Calderdale and Huddersfield NHS Foundation Trust Cardiac Rehabilitation / Heart Failure Team Leader £37,338 to £44,962 a year, pro rata.
