AS PART OF OUR NURSING TEAM YOU WILL: Oversee patient care services Assess new referrals Visit clients at home to evaluate for services Develop care plans and introduce home care aides to new clients Supervise and support our home health aide staff in the field Provide medication management for clients Help ensure that medication is taken safely and as directed. Develop and implement a Plan of Care related to medication, includes calling in refills and contacting the MD with questions as needed. WORK SCHEDULE / COVERAGE AREA Weekdays: 2-4 days/week Visit clients in the Metro North, Merrimack Valley, North Shore and Cape Ann communities Requirements A current Massachusetts RN/LPN license with at least 2 years nursing experience Strong clinical and computer skills Excellent communication skills A TEAM player that is self-motivated with the ability to function autonomously. The ability to prioritize and demonstrate problem solving skills. Previous home care experience is preferred;
