Housing First Case Coordinator

and help provide life-changing support to those who need it most. This exciting part-time opportunity offers the chance to work in a collaborative, compassionate, and solutions-focused environment where you will be at the forefront of empowering individuals to secure stable housing and achieve long-term well-being. Key Responsibilities: Client Support:

Work directly with individuals experiencing homelessness to assess their needs and connect them to housing solutions in alignment with the Housing First model. Case Management:

Develop personalised housing plans, coordinate resources, and provide ongoing support to ensure clients are able to maintain their housing successfully. Collaboration:

Work closely with community partners, including mental health professionals, substance use teams, and healthcare providers, to ensure clients receive comprehensive support. Crisis Intervention:

Provide immediate assistance and de-escalation support to clients in crisis, ensuring their safety and well-being. Documentation & Reporting:

Maintain accurate and timely case notes, assessments, and other required documentation to track client progress and service delivery. Advocacy:

Act as a champion for clients, advocating for their needs with service providers and ensuring they have access to the resources they deserve. Community Engagement:

Support clients in accessing community-based services, including job training, healthcare, and mental health services, to promote long-term stability. Why Join Us? Impact:
