Overall Objectives of the Post: Our Adults and Integrated Care Service is responsible for a range of services and activities that support people with social care and health needs, including carers. The approach used by the service puts more focus on supporting people to remain independent and to live better lives. Our Let's Talk Together approach, based on 3 conversations, puts the person at the centre of their decision making, as experts in their own lives.
Key Tasks of the Post: Be responsible for the coordination and management of a daily position statement and circulate it daily, or as and when required in times of surge, to all partners. Use active listening skills, observation, and communication to build relationships with all teams, partners, and providers. Be an ambassador for South Tyneside Social Services promoting good working relationships in line with South Tyneside's PROUD values. Confidently and professionally challenge decisions, delays, and queries which impact the flow of hospital discharge. Participate in effective solution-focused discussions to manage flow, escalating to the Hospital Discharge Manager where there are significant delays or risks. Ensure there is access to care and support if needed on the day of discharge and work with partners to reduce delays and promote wellbeing. Work closely with health, housing, and voluntary sector colleagues to ensure resources are used effectively across the system. Multi-disciplinary working will be critical to the success of this role. Work in accordance with safeguarding policies, safeguarding individuals in a way that supports them in making choices and having control. Ensure that the 'Making Safeguarding Personal' ethos is at the heart of all practice. Develop an awareness of services available at home and understand the roles and accountabilities of other agencies and professionals in order to refer, liaise, consult, and work effectively to meet identified needs. Oversee and manage daily tasks, phone calls, and email enquiries received into the Hospital Discharge Team. Manage daily operational queries including those of a complex or potentially contentious nature, in a way that is customer-focused and lends itself to positive outcomes. Maintain resilience while working under pressure in a fast-paced service. Work autonomously as well as within a team. Be flexible to adapt to change as the service demands. Have due regard to the Council's current management arrangements for Data Quality, ensuring data collected is accurate, valid, reliable, relevant, and complete. Support the management of data quality assurance as directed by the Hospital Discharge Manager. Prepare and present reports of a statutory and non-statutory nature. Engage in peer support and challenge. Champion a commitment to equality and diversity throughout the service and implement relevant anti-discriminatory policies in all aspects of the service's work. Identify own learning and development needs to continually improve upon practice and support CPD and engagement in personal annual reviews. Contribute to the ongoing improvement and development of Adults and Integrated Care.
An enhanced DBS is required for this post.
