Working alongside Hospital Discharge staff, care homes, individuals, families & carers, you will undertake care coordination, review, and practical interventions within the Renfrewshire area to support discharge planning. This role is based within the Hospital Discharge team, as part of RHSCP Adult Services. Primarily based within Council Headquarters, however also within the RAH where required.
Responsibilities: Work closely with care homes to negotiate and plan hospital discharges in a timely manner. Support and promote the partnerships Delayed Discharge agenda. Undertake assessments and reviews for older adults within their own home or within a care environment. Manage demanding and complex services and handle customer enquiries. Develop effective working relationships with managers, staff, and other stakeholders. Demonstrate effective time management and ability to set priorities. Use IT packages and manage data through electronic systems. Work as part of a team and prioritize your own workload.
Qualifications: SVQ Level 3 in Social Care is essential or be willing to work towards it. HNC in Health & Social Care is desirable. Experience with Eclipse is a plus.
Full training and support will be given to ensure that you are able to maintain the department's commitment to continuous improvement.
