Conducting Discharge to Assess reviews, assessing individuals in their own homes or care settings to determine ongoing care and support needs CHC MDTs:
Participating in Continuing Healthcare Multi-Disciplinary Team meetings to assess and review eligibility for CHC funding and to ensure appropriate care plans are in place Hospital Avoidance:
