We are excited to offer X2 substantive part time positions at 22.5 hours each per week (3 days) to join our hospital palliative care team as a cross site Clinical Nurse Specialist based at both the Royal Derby Hospital (RDH) and Queens Hospital Burton (QHB). The hospital palliative care team provide specialist palliative care support for patients and those important to them across University Hospitals of Derby and Burton. This is delivered via face to face contact on wards, or by supporting the wider clinical team in decision making regarding best treatments. In addition to patient care, a key element of this role will be to deliver education (both formal & informal) to members of the multi-disciplinary team and to drive service improvement. This includes weekend and bank holiday working. Main duties of the job

You will hold a joint caseload of patients with various diagnoses who have complex palliative care needs. You will work alongside their extended care team providing expert advice and support in order to maintain the patients' quality of life and allow them to be cared for in a place of their choosing. You will have demonstrable experience at band 6 or above and have extensive experience of working with patients with complex palliative care needs. An excellent knowledge of symptom management is essential, as is the ability to teach and support others. You will require excellent communication skills and have a sensitive and empathetic approach.
