The post holder will be the key person identified for providing cancer information and support, to enable people to navigate the health and social care system and make choices that are best for their cancer and their life. The aim of the role is to make sure that people living with cancer can access the right support, at the right time. The post-holder will take an active role in contributing towards the development and implementation of cancer survivorship initiatives enabling people affected by cancer to navigate the health and social care system and become self-supportive, i.e., coordinating health and well-being clinics to enable independence and support self-management as appropriate. The post holder will co-ordinate care by providing a single point of access for cancer patients including rapid re-entry into the system for those people identified as having urgent or specialist needs. Qualifications, Specific Experience & Training

NVQ level 3 qualification in Health or Social Care and relevant experience Excellent computer/keyboard skills, ECDL or equivalent, proficient in MS Office 5 GCSE qualifications with C or above including Maths and English Counselling qualification Basic Cancer Course Experience coordinating a patient workload Additional training to Foundation degree/diploma level or equivalent through short courses Knowledge and Skills

Minimum 3 years' experience of working in a health or social care setting Minimum 2 years' experience of working with cancer patients Awareness of the individual needs of people affected by cancer Experience of responding to the support and information needs of others Previous experience as Senior Support worker Experience of working with patients specific to tumor group Understanding of Macmillan Cancer Support and its role across the UK. Awareness of Improving Outcomes; A Strategy for Cancer and other national cancer policies Knowledge of relevant cancer treatments, interventions, and terminology. Experience of working without supervision using own initiative Personal Attributes
